The provider will change from CHCP to CGL wef 1st October 2019, current providers will have received the following letters and will be required to return a signed copy of the continuation letter in order to continue with the service:
A new Varenicline PGD will be sent to you in due course and that will reflect the current PGD which is in use. Please re-read and sign the new PGD once available.
Please refer to your original signed service specification. (Contract extended to 30/09/19) Extension letters sent to contractors by CHCP.
From 1st Oct 2018, the PGD for varenicline will be modified to incorporate clients who start their Varenicline treatment in the acute hospital (Castle Hill & Hull Royal Infirmary). Contractors will receive the amended PGD from CHCP shortly, all Pharmacists providing this service must sign and retain a copy of the PGD within the pharmacy. The revised PGD will also be added to the PharmOutcomes platform.
Read the Service Specification and complete the necessary training/CPD to provide the service. All Pharmacists providing this service must sign and retain a copy of the PGD within the pharmacy.
