Medication Record Charts for Carers Service Hull & ERoY

Aims and intended outcomes of the service:

For community pharmacies to support domiciliary care workers in the safe administration of medication by preparing medication record charts (MRC) for patients under their care.

Charts will be created by the pharmacy adding duplicate labels of dispensed medication to the chart as per the service specification.

The social services team will contact the pharmacy following completion of a fullers assessment of a patient  to enquire if the pharmacy is willing to provide the MRC service to the patient. If agreed the social services team will then send a copy of the fullers report to the pharmacy.

Service Period:

April 2025 to March 2029

Claims:

All claims data to be submitted via the PharmOutcomes platform. All payment claims will be raised via the PharmOutcomes automated invoicing mechanism on the 4^th day of the month.

Training/Accreditation
