Medication Record Charts for Carers Service Hull & ERoY

Aims and intended outcomes of the service:
For community pharmacies to support domiciliary care workers in the safe administration of medication by preparing domiciliary medication record charts (DomMAR) for patients under their care.
Charts will be created by the pharmacy adding duplicate labels of dispensed medication to the chart as per the service specification.
The social services team will contact the pharmacy following completion of a fuller’s assessment of a patient, to enquire if the pharmacy is willing to provide the DomMAR service to the patient. If agreed, the social services team will then send a copy of the fuller’s report to the pharmacy.
Service Period:
April 2025 to March 2029

Claims:
All claims’ data to be submitted via the PharmOutcomes platform.
All payment claims will be raised via the PharmOutcomes automated invoicing mechanism on the 4th day of the month.
Training/Accreditation:
No additional training required but please read service specification.
Requesting Charts:
From the 19th of June 2026, you will no longer be able to request Medication Record Charts from the ICB via the previous ordering form.
New Process:
Following support from the Primary Care Strategic Executive to explore a number of options, a recommendation was made to consolidate the printing of Medication Record Charts to include direct, automatic delivery to participating pharmacies.
Pharmacy providers no longer need to request a supply of charts, instead pharmacies will receive an automatic delivery of one years’ supply of charts during week commencing the 22nd of June 2026. (please store these safely).
The number of charts provided to each pharmacy will be based on the number of charts claimed for between June 2025 and May 2026.
A contingent supply of charts will be available from Humber LPC should you require additional charts beyond your annual delivery.
