Humber and North Yorkshire Joint Formulary
The former Humber and North Yorkshire formularies have now been brought together into a single, system-wide formulary.

This means:

- ✅ Clearer, more consistent prescribing guidance
- ✅ A shared approach across Humber & North Yorkshire
What you need to know
We know this isn’t perfect yet – and that’s okay. The formulary will continue to be reviewed and aligned over time as part of our ongoing system approach.
Please use the new HNY formulary from now on; the former legacy formularies will NOT be updated.
We have a robust and transparent process for formulary management:
- Applications for changes to formulary can be submitted to the Medicines Formulary Group (MFG). Please email: hnyicb.mfg@nhs.net, and we can share the relevant information required for an application.
- Requests are considered and, where appropriate, changes/ updates are shared for a 4-week system-wide consultation via https://humberandnorthyorkshire.org.uk/area-prescribing-committee-apc/apc-consultations/
- Final decisions are made through established governance routes, via the Humber and North Yorkshire Area Prescribing Committee.
What we expect from ALL prescribers, regardless of setting:
- All prescribers are expected to follow the formulary most of the time, ~90% of the time. Any deviation from the formulary must be supported by a clear, clinical rationale.

- If you are asked to prescribe something that doesn’t feel in line with the formulary or is not clinically appropriate, let us know, and we will follow up and address these requests. Please email: hnyicb.prescribingqueries@nhs.net
